Job Description
Overview:
This is a great opportunity in downtown Portland for candidates with a strong clinical background (Medical Assistant or Certified Nursing Assistant), who are looking for a less physical role to move into.
The Elder Life Specialist handles evaluation and enrollment of elderly patients who are at a high risk of developing complications. This position also involves training and assigning hospital volunteers to deliver patient interventions.
What you will do:
- Perform daily thorough geriatric intake assessment and enrollment of new patients into the program. Screening procedure includes medical review, patient/family interview and nurse interview along with EL Nurse Specialist.
- Update treatment plans daily and assign volunteers to work with particular patients and define for them appropriate interventions.
- Implement treatment plans and therapeutic activities which will meet individual and group treatment, under supervision of the clinical staff.
Responsibilities:
- Contribute to essential documentation requirements which may encompass screenings, evaluations, progress notes and discharge summaries for all assigned patients.
- Work in conjunction with other team members in formulating and implementing treatment plans and treatment approaches for assigned patients.
- Assume responsibility for patient safety when working with the patients.
Requirements:
- A strong clinical background (Medical Assistant, Certified Nursing Assistant, etc.) to navigate patient charts.
- Strong organizational skills
- Ability to interact with elderly patients in a friendly manner.
- High school education required, degree in a healthcare related field is preferred.
- License/Certifications: Certified as Nursing Assistant by State of Maine, or eligible for certification as a Therapeutic Recreational Specialist (CTRS) as administered by NCTRC, or eligible for LSW with focus on elder care.
- Previous work in elder care preferred but not required.
Job Type: Full-time
Salary: $22.00 per hour
